I don't know if you have tried the following:

-Social Services - if she has low income she can qualify for some assistance. Such as, Medicaid, Food Stamps, Incontinence supplies, etc.

-Dept. Of Aging- also has benefits; such as, personal care assistance (if she qualifies), access to adult day care facilities, and much more.

Social Security Admin- If your Grandmother can no longer take care of her finances you will need a letter from her physician indicating her illness and stating that she can not handle her financial affairs. Take this letter along with both of your valid driver licenses/IDs and apply for representative payee. This way you can handle any monies from Soc. Security. Medicare.

You should also get a Power of Attorney. You'll need this to handle any medical and other financial issues (life insurance, bills, etc.) that may occur.

You can get the Power of Attorney (POA) done at her bank (take her along if she can travel) - While you are there, have you and your brother's name added to POA as well as her bank account. Please note that Social Sec Admin will not accept the POA.

Hope this helps.

Sincerely,
Been there and currently living it!
